Our Team


Jeremy graduated from Nazareth College of Rochester in 2001 with a Master's Degree in Physical Therapy. In 2004, he graduated from Daemen College in Buffalo, with a Doctor of Physical Therapy Degree. He is also certified in Mechanical Diagnosis and Therapy of the Spine.

Jeremy is very involved with both the state (New York Physical Therapy Association) and national (American Physical Therapy Association) professional organizations.

Megan Maple City Physical Therapy


Megan graduated from Nazareth College of Rochester in 2005 with a Master's Degree in Physical Therapy. Later, in 2008, she graduated from Daemen College in Buffalo, also with a Doctor of Physical Therapy Degree.

She is also certified in Vestibular Rehabilitation. Megan is very involved with both the state (New York Physical Therapy Association) and national (American Physical Therapy Association) professional organizations.


Nichole is the Director of Practice Management  at Maple City Physical Therapy. Her position includes starting new quality initiatives at Maple City Physical Therapy, as well as managing office functions such as billing, scheduling, and claims management.

"I enjoy interacting with the patients and helping them answer insurance questions," said Nichole.

Nichole has an Associates Degree in Computer Information Systems from Alfred State College.

She is in integral part of day-to-day operations at MCPT.

Tyler is a 2011 graduate of Naples High School and received a Doctor of Physical Therapy from Daemon College. 

“I like having the chance to work with people,” said Tyler. “I enjoy the satisfaction of helping people return to function — whatever that may be. Whether it is sports, or their daily mile walks. That is the reward for me.”

Tyler has a special interest in helping children with their physical therapy needs and currently treats children at Andover Central School.

Tyler is currently working on his certification in Mechanical Diagnosis and Treatment of the Spine.

Tyler works at the Hornell and Andover offices, as well as providing services at Andover Central School.

Jessica received Doctor of Physical Therapy from Gannon University. A 2009 graduate of Wellsville High School, she splits her time between the Hornell and new Andover office. Jessica also interned with us in the summer of 2015.

“I really love it here,” said Jessica. “I like the people I work with and all of the patients.”

Jessica is certified in LSVT BIG which is a specialized program of examination and treatment for patients with Parkinson's Disease. She is also certified in evaluation and treatment of patients with dizziness and balance problems.

Jessica works at the Hornell and Andover offices, as well as providing services to preschool children in Allegany County.

Brett is a 2011 graduate of Bath-Haverling High School, 2017 graduate of Nazareth College, Brett works with the whole body, but has a particular interest in sports performance, strength and conditioning.

“Everybody here is great, very warm and welcoming," said Brett. "I feel like I have worked here for years"

Brett is certified in cupping therapy, which can help patients with muscular and myofascial  pain disorders.

Brett is Certified in Evaluation and Treatment of the Fitness Athlete, and enjoys working with athletes at all skill levels.

Myranda is a recent graduate of Gannon University in Erie, Pennsylvania where she earned her Doctor of Physical Therapy degree. She also has a LSVT BIG Certification to treat Parkinsons Disease.

She says she enjoys working with Parkinsons patients because of the joy on their faces when the treatment improves their condition.

Myranda shadowed Jeremy and Megan while she was still in school, and she now feels right at home.

“Things have changed a lot, the practice has grown so much over the years,” she said. “I love it here, it is very busy, but it feels like family. That’s what makes it really nice. I hope that I can bring a positive energy to everyone. I am still getting used to it here, but I hope to quickly become part of the family.

Myranda works at the Hornell and Andover offices, as well as providing services at Belfast Central School.

Jen graduated from Genesee Community College in 2002 with an Associate's Degree, allowing her to be a Physical Therapist Assistant. Jennifer brings a work history from multiple environments, making her a very well-rounded member of our clinical team.

She holds a certificate in gerontology, and has a special interest in pediatrics.

"I like the variety of diagnoses that I work with," says Jen. "The ability to help people improve their lives within a short period of time is very rewarding. Having coworkers and bosses that care about you as as a person, not just as an employee or a patient is a big reason I like it here. It is a great family atmosphere."


Heather earned her Associates Degree in Applied Science and Physical Therapy Assistant and has joined the staff here as a permanent member after spending a summer as an intern at Maple City Physical Therapy.

Her favorite part of her job is interacting with the patients. "I love to get to know the patients, find out who they are and what they do," she said. "I like to get out to meet new people. People have some really awesome stories."

"I like that I get to work with a variety of conditions rather than focusing on one specific ailment."

Samantha is a Hornell High School and Herkimer County Community College graduate. Sam works as a Physical Therapist Assistant. Once injuries are diagnosed and a treatment plan is developed, she helps to implement the plan.

Her favorite part of the job is the satisfaction when she helps others achieve their goals. “At first people come in feeling down, then to watch them walk out two months later like nothing ever happened is nice. It’s great to come in and enjoy what you are doing and who you are working with.”

Samantha works at the Hornell and Andover offices.

Kaylee, Physician Liaison and Patient Engagement Coordinator, will be your point person when you come to Maple City Physical Therapy. Kaylee will reach out to physician offices to better meet both their needs and the needs of their patients. She will be working to make the patient experience of each person as positive as possible! She answers calls, schedules appointments, and answers any other questions new patients may have. Located in the Hornell office, Kaylee has a Bachelor’s Degree in Exercise Science. “I really like working here,” said Kaylee. “My coworkers make it a fun place to work.

Kellie is Maple City Physical Therapy’s Patient Engagement Coordinator and Physician Liaison. She will be engaging patients and assuring their needs at MCPT are being met, and progress is being reported to their Primary Care Physicians. She will also be reaching out to local physicians to share patient experiences.

Kellie truly believes in Maple City Physical Therapy’s mission. "You come here and feel like you are really taken care of," she said. "(The staff) is Interested in what is going on with you and how they can help. This is the kind of medicine people should really do more of. There is so much that can be done — you don’t have to pop pills. I know I am in the right place. I can see the value here.”

Kellie has an Associates degree in Health Information Management from Alfred State College.

Hailey Schwartz of Canisteo is the smiling face behind the front desk greeting you as you walk in the door. As the office assistant, she will be answering your phone calls, checking people in and working with insurance.

A Canisteo native, she is also a full-time student at Alfred State College where she is majoring in Technology Management. So far she says working at Maple City Physical Therapy is challenging and rewarding. “I like everyone here,” she said. “The atmosphere is always really fun and outgoing.”

Matt Grosso, a 2019 Alfred University graduate is the Athletic Trainer at Maple City Physical Therapy.

“I did my internship here my senior year, and I really enjoyed it,” said Matt.

Matt will be working with the Hornell High School athletic teams in a preventative capacity. This will include rehabilitation, emergency on-field care, bracing and taping of the athletes. He will be working with athletes from the onset of an injury through to the end. He will be present at many of Hornell’s home games.
