Tyler is a 2011 graduate of Naples High School and received a Doctor of Physical Therapy from Daemon College.
When not working, Tyler enjoys running, listening to music, reading and hiking. He also trains for 5Ks and participates in as many as he can every year.
He is a certified McKenzie trained therapist (MDT certified) and has special training in evaluating and treating problems with the spine.
His favorite part of the job is the “the friendly environment and awesome people I get to meet.”
If you could switch lives with anyone in the world for one day, who would it be?
It would have been when Neil Armstrong walked on the moon. In my opinion, that moment would have been larger than life and so incredible to experience.
Tyler works at the Hornell office.
Kaylee works to assist clients in the gym to help meet their fitness goals, as well as working to complete various office tasks for Maple City Physical Therapy. In the Hornell and Andover offices, she answers calls, schedules appointments, and answers any other questions new patients may have. Kaylee has a Bachelor’s Degree in Exercise Science. “I really like working here,” said Kaylee. “My coworkers make it a fun place to work.”
Samantha is a Hornell High School and Herkimer County Community College graduate. Sam is the Marketing and Clinical Coordinator as well as a Physical Therapist Assistant. As a marketing and Clinical Care Coordinator, Sam is responsible for reaching out to doctors and developing marketing plans to attract new patients. She also verifies insurance eligibility and works closely with patients to assure they are enjoying their experience at MCPT.
“Outside of work I enjoy spending time with family and friends, as well as doing crafts, being outside, and watching football.”
Her favorite part of working at MCPT is getting to help people achieve their therapy goals, and getting to know new people. “At first people come in feeling down, then to watch them walk out two months later like nothing ever happened is nice. It’s great to come in and enjoy what you are doing and who you are working with.”
Samantha works at the Hornell and Andover offices.
Tiffany is originally from Wellsville, NY and graduated from Scio Central School in 2014. She received her Doctorate in Physical Therapy from Clarkson University in May 2021.
Tiffany primarily works in our Andover office with patients of all varieties. She is working on her specialty in Pelvic Health, evaluating and treating these patients in our Andover office. In her time thus far at Maple City Physical Therapy, she really like the culture and how dedicated the therapists are to helping patients achieve their goals. “The physical therapists are always searching for ways to help the patient get back to the things that are most important to them.”
In her free time, she enjoys reading, painting, and spending time with her dog, Sadie Mae. In addition, she nurtures a plant family that she keeps.
Kylie is originally from Wellsville, NY. She attended Alfred University receiving a Bachelor’s Degree in Athletic Training. She is now attending the University of Idaho, working on receiving her Doctorate in Athletic Training. Kylie works to improve athletic-related injuries through prevention, treatment, and rehabilitation. Her favorite part of what she does is helping athletes to regain their health, utilizing a wholistic approach. Her favorite part about working at Maple City Health and Fitness thus far is the sense of community. In her spare time, she enjoys hanging out with family, playing cards, reading, and practicing yoga.
Jessica is originally from Auburn, NY. She received a Bachelor’s Degree from Alfred University and a Master’s Degree from The University of Central Missouri. She works in our local school districts as an athletic trainer, helping to keep our local athletes healthy and safe. Her favorite part about working at Maple City Health and Fitness so far is working with the wonderful staff and enjoying all the people she has met. In her spare time, she enjoys running, working out, outdoor activities, and farming. The best part of her job is having the ability to take care of young athletes and see them be able to perform at the best of their abilities.
Mackenzie is originally from Canisteo, NY. She attended and graduated college from both SUNY Morrisville and Alfred State College, with a Bachelor’s in Business Administration in Technology. Mackenzie works on scheduling patients in an efficient manner, as well as
answering patient questions and phone calls. In addition, she is helping with the billing process, as well as patient insurance authorizations.
Thus far, she has enjoyed the positive working environment at Maple City Physical Therapy and participating in a team-oriented workplace.
Her hobbies include spending time with her family, and spending as much time in Vermont as possible.
The best part of her job thus far has been seeing patients recover and make progress, as well as hearing their stories when they check in for appointments.
Nichole is Chief Billing and Compliance Officer at Maple City Physical Therapy where she has worked since 2016. Her position includes starting new quality initiatives at Maple City Physical Therapy, as well as managing office functions such as billing, scheduling, and claims management.
When not working she enjoys watching her kids play sports and traveling. Her favorite place on earth is Mt Holly, Vermont. She also enjoys volunteer work. “Working at MCPT I’ve had a ton of opportunities to partner with other local organizations. The Hornell YMCA, Canisteo Rotary, and HPG always have events going on that we can contribute to in some way.”
What is your favorite part about working at MCPT? We genuinely enjoy each other and take care of each other it’s also great to have so many patients that have become our friends.
Mitchell grew up and currently lives in Palmyra, NY. He went to Syracuse University for a History Degree and then attended Bryant and Stratton College for an Associate’s Degree to be a Physical Therapist Assistant.
Thus far at Maple City Physical Therapy, he has enjoyed the family atmosphere, as well as the team environment that provides patients with the best possible care. In his spare time, he loves following the Liverpool Football Club, and has been lucky enough to see them in person on 3 occasions. He also enjoys going to his children’s sporting events, watching Cobra Kai, and playing tennis with his daughter.
His favorite part of being a physical therapist assistant is seeing the positive impact he has on other people’s lives on a daily basis which makes his work extremely rewarding.
Shanna is from Andover, NY and was recently hired as a Clinical Director in our Andover office. Shanna earned a Master’s Degree in Physical Therapy from Utica College and a Doctorate in Physical Therapy from Daemen College. Shanna has 20 years of clinical experience, is certified in wound care and evaluation, and also has specialized training in mechanical diagnosis and treatment of the spine.
Thus far in her time at Maple City Physical Therapy, she has enjoyed the team-oriented atmosphere, communication level, and welcoming environment. Her favorite part of being a physical therapist is working with patients, seeing their improvements and how it helps them functionally. Shanna does her best to connect with patients and to find a way to joke around and have fun with them, while encouraging them to do better and be better than they ever thought they could.
In her free time, Shanna enjoys camping, kayaking, fishing, and any activity or adventure with her family.
Myranda recently received a promotion to Senior Physical Therapist I, Community Outreach and Contract Services. In this role, Myanda helps to coordinate and plan volunteerism in our communities, as well as assisting in training and administration of our contract services at Maple City Physical Therapy.
Myranda is a graduate of Gannon University in Erie, PA. She has been working at MCPT since the day after she graduated college in 2018. Myranda works at the Hornell and Andover offices, as well as providing services at Belfast Central School.
A member of the events committee for the Wellsville Area Chamber of Commerce, Myranda also loves spending time with her husband and their dog when she is not working. “I also enjoy running and working out. During football season I like to watch the Buffalo Bills, we have season tickets!”
I am certified in LSVT BIG which is a treatment for Parkinson’s disease. I am also becoming certified in Hand Therapy. I also provide physical therapy services at Genesee Valley Central School.
What is your favorite part about working at MCPT?
I would have to say just the bonds that are formed between the therapists and their patients. We do not just treat the injury; we treat the patient individually and you end up forming bonds with them throughout the time they are here! I also love that we provide patient engagement opportunities and contests that help them feel appreciated and involved!
If you could switch lives with anyone in the world for one day, who would it be?
I do not really have a specific answer to this question. I would say it could be anyone that has had an experience or life experiences that I have not had. Someone from another place would be exciting but also someone like my grandma who has had a lot of life experience would also be nice since she has accomplished and experienced so much!
Lyndsey is originally from Middleburgh, NY. She attended Alfred University and graduated with a degree in Athletic Training, minoring in Exercise Science.
Lyndsey works to ensure that athletes are able to participate in sports safely and effectively. As part of that, she regularly communicates with PT’s, physicians, parents, and students to ensure the best care is provided to all athletes.
Thus far, her favorite part of being at Maple City Physical Therapy has been interacting with the physical therapist’s in the clinic setting, as well as getting to know her fellow athletic trainer’s on a personal level.
In her free time, she enjoys hiking, running, baking, fishing, and playing basketball.
Her favorite part of her job thus far has been enjoying watching her athletes perform at a high level of competition and ensuring their safety.
Leslie is originally from Swain, NY. She graduated from Lock Haven University with a Master’s Degree in Athletic Training.
She works to help athletes return to play after injury, and assists in preventing injuries with proper training techniques.
Thus far at Maple City Physical Therapy, she has enjoyed working with the physical therapists, physical therapist assistants, and athletic trainers who specialize in different areas of health and wellness.
She enjoys hiking, cooking, and spending time with family and friends in her spare time.
Her favorite part of what she does is watching athletes be able to confidently return to the sport they love after overcoming an injury.
Hannah is originally from Oxford, Michigan. She attended college at Houghton College and Washington University in St. Louis where she received her Doctorate in Physical Therapy.
In her time at Maple City Physical Therapy so far, she has enjoyed the friendly nature of her coworkers. In her spare time she enjoys hiking at Letchworth State Park, as well as spending time with her husband and cat.
Her favorite part of being a physical therapist is getting to help patients, assisting them in meeting their goals, and seeing them return to doing the things that they love.
Jaclyn graduated from Arkport Central School in 2011, and received an Associate’s Degree from Corning Community College in Humanities and Social Sciences. Jaclyn works in the Bath and Hornell offices, and is responsible for answering patient phone calls, scheduling patients in a timely and convenient manner, as well as answering any questions you may have about our facility. Jaclyn also works on insurance benefit checks and the new patient intake process.
Jaclyn says her favorite thing about working at Maple City Physical Therapy is that she loves the family atmosphere and assisting our patients in meeting their PT needs. Her hobbies include spending time with her children, enjoying the outdoors, and living each day to the fullest.
The best part of her job is watching the progress that patients make, and seeing how far everyone comes during their time with us.
Theresa is originally from Greenwood, NY. She attended Bryant and Stratton, completing the Medical Receptionist Program there.
She feels that the most important part of her job is making our patients feel welcome from the time they schedule until graduation day. Thus far she has enjoyed how the employees at Maple City Physical Therapy work together as a team, making her days pass quickly because “we’re all in it together”.
In her free time, she enjoys riding her side-by-side, spending time with her family and dogs.
Her favorite part of her job is greeting patients, and helping the therapists with anything that will make their day go easier!
Bryan Cheskiewicz is new to Maple City Physical Therapy, and the Hornell area. The 2014 Starpoint High School graduate (Lockport) received his Doctor of Physical Therapy in 2020 from Daemen College. He has nothing but positive things to say about his time at MCPT so far, “It’s going well, people are nice here, I am enjoying it so far,” he said.
“My interests outside of work include; playing with my dog named Layla, playing in sports, playing cards with friends, and watching the Bills.
As of right now I do not have any specialties, but I am interested in treating patients with spine injuries and love to work with geriatrics.
My favorite part about working at MCPT is working with patients from this area and getting to know the community in Hornell.
If i could switch lives with one person in this world for one day, it would have to be Terry Pegula because I am a huge Bills and Sabres fan.
Andrea is the newest face at Maple City Physical Therapy.
A 2014 graduate of Canisteo-Greenwood Central School, Andrea received her Doctor of Physical Therapy in 2020.

“I am really interested in working with the pediatric population, as well as people with balance and gait challenges. Andrea sees patients in the Hornell and Andover clinics, and is the provider of school based therapy services to several local school districts.
Jessica received Doctor of Physical Therapy from Gannon University. A 2009 graduate of Wellsville High School, she splits her time between the Hornell and new Andover office. Jessica also interned with us in the summer of 2015.
“I really love it here,” said Jessica. “I like the people I work with and all of the patients.”
When Jess isn’t at work, she enjoys spending time with her dog, Tuko, and spending a lot of time with my family.
A member of the APTA, she specializes in Vestibular rehab, LSVT BIG and wound care.
What is your favorite part about working at MCPT?
I enjoy the relationships I build with my patients and seeing people improve each day. We have a great patient base.
If you could switch lives with anyone in the world for one day, who would it be?
My dog Tuko, because he is the happiest and loves to cuddle.
Logan Seiferth is one of the first friendly faces you will see when arriving at your appointment at Maple City Physical Therapy. A recent graduate of University of Buffalo, he will be the new Office Assistant. Logan will be responsible for answering you phone calls, checking temperatures, scheduling appointments and doing health checks when you arrive.
During his time working at the office, he has enjoyed the fun atmosphere and getting to know the patients and his fellow coworkers. “Everyone has been very nice and extremely helpful, I enjoy coming to work,” he said. “Everyone is funny, kind, and very good at when they do.
When he isn’t working, he enjoys watching and playing sports, especially the Bills and the Sabers. He is also a bit of a history buff and knows a lot of random facts about the world.
If he could switch places with anyone for the day, he would choose Jack Eichel, “Because he is the captain of the Sabres and the best player in the world.”
Emily Pawlowski is the newest Physical Therapist at Maple City Physical Therapy. A recent addition to the Hornell area, Emily is originally from Williamsville, NY. She earned her Doctor of Physical Therapy from Nazareth College. She particularly enjoys working with people with neurological issues.
“I really like the atmosphere (at MCPT), it is really warm and inviting.”
Mike is a 2014 graduate of Hornell High School. After high school he went on to earn his Doctor of Physical Therapy from Daemon College in 2020.
“I think coming back to work in my hometown is really cool,” said Mike. “The patients have been great and courteous, letting me learn their stories.”
Mike participated in a job shadow program at MCPT his freshman year of college and is impressed with both the expansion of the facility and its staff. “They have taken their idea of what physical therapy should be in the area, and they ran with it.”
Mike said that one of the reasons he chose MCPT is because giving back to his community is important to him.
I like to do a lot of outdoor activities such as kayaking, hiking, and playing sports.
I love being able to help give back to my community and being able to be part of such a great team.

Brett is a 2011 graduate of Bath-Haverling High School, 2017 graduate of Nazareth College, Brett works with the whole body, but has a particular interest in sports performance, strength and conditioning. Brett is Certified in Evaluation and Treatment of the Fitness Athlete, and enjoys working with athletes at all skill levels. He is also a member of the American Physical Therapy Association.
“Outside of work I enjoy working out, riding my bike, and hiking. I also enjoy riding my motorcycle and snowmobile.”
What is your favorite part about working at MCPT?
My favorite part about working at MCPT is the tight knit relationship we have as co-workers and the outstanding community that we service.
Megan graduated from Nazareth College of Rochester in 2005 with a Master’s Degree in Physical Therapy. Later, in 2008, she graduated from Daemen College in Buffalo, with a Doctor of Physical Therapy Degree.
She is also certified in Vestibular Rehabilitation.
